This statement will explain to you what I do with your personal information.
- What personal information do I store? Your name, phone number and email address are stored electronically in my mobile phone and in my Outlook email account, both of which are password protected. These are not linked to any other personal information you give me, and are not shared with any third party, they are used solely by me to communicate with you regarding appointments etc. Other personal information you give me during the counselling session is kept on hard copy only, is anonymised and securely stored in a locked cabinet.
- How long is information kept for? I keep a record of your attendance at counselling for 5 years which is a requirement of my insurer (5 years from 18th birthday in the case of a child or young person). This record is securely stored as above. I have a Clinical Will in place to ensure that, in the event of my sudden death, any personal information held by me will be destroyed by my Clinical Supervisor.
- Your rights: Under data protection law you have certain rights: you have the right to know and to access the information I hold about you, and the reasons why I hold it. You have the right to have any inaccuracies rectified, and you may also have a right to request I destroy your information, subject to approval by my insurers.
- Registration with ICO: I am registered with the Information Commissioner’s Office; you can contact them should you have any cause for complaint about how I store or use your data.
